from my stand point (I've been in warehousing the past 6 years.) this type of accident can happen very easily. looks like the have 25' uprights with 12' bays. the biggest thing i can see is that they have single upright isle configuration that would make it more likely to tip. if you take into account that guy hit a hollow piece of steel with a 10-16 thousand pound propane forklift I'm surprised he didn't get further then he did. i see pictures of this stuff all the time at work, some companies go bankrupt from the aftermath.

you told me it was an eye exam. boy was that awkward. almost as awkward as the time you went to the nambla convention thinking it was cosplay. but on the plus side the sailor moon costume was spot on
